Patient Portal Information

We offer a patient portal through Valant, our electronic medical records provider. This service is offered as a courtesy to patients and there is no fee associated with using the portal. The portal is intended to provide a secure method for patients and providers to communicate with each other and for you to be able to access your billing, contact, and scheduling information. Portals can be a valuable communication tool; however, they also have some risks and conditions that you should consider.
How it works:
A patient portal is a secure web page that uses encryption to keep unauthorized individuals from accessing your information and communication with your providers. Secure messages can only be read by someone who has the correct password to log into the portal site. The portal allows you to:
• Review and update your demographic information
• Complete intake forms and assessment measures
• Receive educational materials
• View and print your invoices
• Send messages to your identified provider
• Receive reminder calls, texts, or emails about your appointments
• Additional functionality may be added in the future
Protecting your Private Health Information:
Patient portals are designed to make sure that unauthorized individuals cannot access your information, even when it’s in transmission. No communication system is completely perfect, although we will do our best to maintain electronic security. In order for us to do that, we require two things from you: (1) the correct email address, and (2) for you to keep your password secure.
Types of Communication:
The portal is not intended as a means to communicate about emergencies, nor is it intended for detailed communication about your treatment or diagnosis. You may use the portal to request an appointment, to reschedule a meeting, to ask for clarification about a homework assignment, or similar non-emergent questions.
Tips and Reminders:
• We strive to ensure the information in your records is accurate. If you identify any discrepancy in your demographic information, please let us know as soon as possible.
• If you lose your password or username, or if you think another person may have learned your password, you may request a new one through the web portal.
• Always remember to log out and close your browser when you are finished accessing the portal. This prevents someone else from accessing your personal information.
• Never use a public computer to access the patient portal.
• We reserve the right to terminate or suspend use of the portal if needed.
Additional Considerations for Minors and for Adult Patients With Guarantors:
We are only able to add one email address per patient for the portal. There are two important considerations in deciding whose email address to use: (1) how the guarantor (i.e., the person who is paying for services) will access the patient’s invoices, and (2) who will be notified about a questionnaire or measure the patient needs to complete. Please note that the portal account holder will be able to read the name of the questionnaire or measure; however, s/he will not be able to read or access the patient’s responses to the questionnaire.
